Add ability to view documents within a form

I'm currently working on a document acknowledgement system for our company and we'd like to be able to show a document in a Nintex Form on Office 365. I've attached a mock-up of what we'd like to see. It should be able to handle showing any Office document or a PDF. If we could also require that the document be scrolled through completed before the fields below it became available, that would be even better.
